When completing your application form please provide us with as much information as possible. Please complete one application form per applicant. Once complete please email a signed copy to admin@purpleoak.com.au or drop it into us at Shop 2/466 Mulgrave Road, Earlville.

Please ensure you have attached the following supporting documentation to your application:

• 100 Points of ID
• Employed – Last three payslips
• Self-Employed – Three months of bank statements, group certificate, tax return or a letter from your accountant verifying income.
• Centrelink Income Statement (If applicable)

In preparation for your vacate, please complete the above Notice of Intention to Leave Form and return a signed copy to your Property Manager – admin@purpleoak.com.au or drop it in to us at Shop 2/466 Mulgrave Road, Earlville.

Please ensure your form is returned to us atleast two weeks before you intend to vacate.
